A Clinical Study on Peptic Ulcer Perforation



Abstract
This report is a clinical analysis of 107 patients of perforated peptic ulcer who were treated at the department of General Surgery in the Korea veterans Hospital from March 1979 to February 1984. And the results were as follows.
1) Among these 107 cases, the perforated gastric ulcers were in 29 cases (27. 1%) and the perforated duodenal ulcers were in 78 cases (72.9%).
2) - In age distribution, the highest incidence was 4th decade, and 5th decade was followed. 3) The Sex distribution was 101 in male and 6 in female with a ratio of 16.8: 1. 4) The seasonal distribution was high in spring and autumn.
5) The duration of characteristic peptic ulcer symptom prior to perforation was, most
common for 1-3 years by 24 cases (22.3%), but 11 cases (9.8%) had no previous ulcer
symptom.
6) By the physical findings on admission, the majority of the cases had abdominal tende-rness (98.2%), abdominal rigidity (93.7%), decreased bowel sound (83.2%).
7) Among the 4 types of blood groups. 0 type was relatively common than others.
8) The Subdiaphragmatic free air shadow was seen in 84 cases (78.4%) and the preoperative duration after perforation was from 12 hours to 24 hours in 40 cases (37.5%).
9) The most frequent location of perforation was in the anterior wall of duodenal first portion with 75 cases (70. 1%) and the size of perforated orifices was less than 5mm in diameter with 65 cases (60.8%).
10) As the operative procedure, vagotomy with pyloroplasty was performed in 33 cases (30.8%), subtotal gastrectomy in 32 cases (29.9%), simple closure in 20 cases (18.7%), vag-otomy with antrectomy or, hemigastrectomy in 20 cases (18. 7%), and parietal cell vagotomy in 2 cases (1.9%)
11) The early postoperative complications were noted in 26 cases (24.2%) and the commo-nest complication was wound infection in 11 cases (10.3%).
12) The mortality rate was 5.6% (6cases) and the most common cause of death was sepsis.
